Who we are
Believe & Achieve Recovery is a licensed addiction-treatment provider located in Glendora, California [legal entity name and address to be confirmed]. This policy covers this website and the intake and coverage-verification forms it offers. It does not replace the notices that govern your clinical records once you become a patient.

We collect only what we need to respond to you and provide care:
- Information you provide: name, phone, email, date of birth, the person you’re seeking help for, and insurance details (provider, policy/group number, and the last four digits of a policy holder’s SSN) submitted through our intake or verification forms.
- Your messages: anything you include when you contact us.
- Technical data: limited, standard information such as IP address, browser type and pages viewed, collected to keep the site secure and working.
- Anti-abuse data: our forms use a bot-protection service (Cloudflare Turnstile), which processes limited technical signals to tell humans from bots.
How we use your information
We use the information you provide to:
- Respond to your request and contact you about treatment.
- Verify insurance benefits and coordinate admission and care.
- Operate, secure and improve this website.
- Comply with legal, regulatory and accreditation obligations.
We do not sell your information
We do not sell your personal information, and we do not share it for cross-context behavioral advertising. We do not use the sensitive information submitted through our forms for marketing.
Health information gets extra protection
Information about your health, treatment or substance use is protected health information. We handle it in accordance with the Health Insurance Portability and Accountability Act (HIPAA, 45 CFR Parts 160 and 164), the federal confidentiality rules for substance use disorder records (42 CFR Part 2), and California’s Confidentiality of Medical Information Act (CMIA). When we share information
We share your information only as needed and as permitted by law — never for sale:
- Service providers who help us operate (for example, secure hosting, or an insurer/clearinghouse to verify benefits), under confidentiality and, where applicable, Business Associate Agreements.
- With your consent, including the consent you give when you submit a form asking us to contact you and verify coverage.
- When required or permitted by law, such as a valid court order, or to prevent serious harm.
Cookies and analytics
This site uses only the cookies and similar technologies needed to function, keep it secure, and understand aggregate usage. We do not use advertising cookies. You can control cookies through your browser settings.
How we protect your information
Form submissions are transmitted over encrypted connections (HTTPS). Sensitive fields such as date of birth and insurance identifiers are encrypted at rest, and access is limited to authorized staff. Marketing content is served separately from any system that holds health information.
How long we keep it
We keep personal information only as long as needed for the purposes above or as required by law and our record-retention obligations, after which it is securely deleted or de-identified.
Your privacy rights
Depending on where you live, you may have the right to access, correct, delete or restrict the use of your personal information, and to withdraw consent to be contacted at any time.

This site and our services are directed to adults and to individuals aged 16 and older seeking treatment. We do not knowingly collect personal information from children under 13.
